Terms and Conditions

Required documents

  • Proof of Age and Identity: A birth certificate or other official document.
  • Proof of Residency: Documents like a utility bill or lease agreement to show you live in the school district’s attendance area.
  • Immunization Records: A Certificate of Immunization.
  • Health Examination: A recent physical exam report (within the last year) from a licensed healthcare provider.
  • Academic Records: Official school records or transcripts, and potentially recommendation forms from previous teachers.

 

School policies

  • Student Conduct and Discipline: You agree that your child must conform to the school’s behavioral standards and may be removed for poor performance or discipline issues.
  • Fee Payments: You agree to pay fees on time.
  • Annual Fee Increases: You acknowledge that fees may increase each year.
  • Liability: You understand the school is not responsible for lost or stolen personal items.
  • Photograph Use: You consent to your child’s photo being used in school publications like brochures or advertisements.

 

Other agreements

  • Program Eligibility: You understand the student must meet age, residency, and other program eligibility requirements to apply.
  • Health Information: You must provide accurate medical information and inform the school of any health problems, allergies, or infectious disease contact.
  • Late Fees: You may agree to pay late fees if required documents are submitted after the deadline.
